Beclomethasone propionate is an inhaled corticosteroid and glucocorticoid receptor agonist being investigated by Tongji University (specifically Shanghai Pulmonary Hospital) for the prevention of radiation pneumonitis in patients with locally advanced non-small cell lung cancer (NSCLC). As a small molecule anti-inflammatory agent, it works by binding to the glucocorticoid receptor to suppress the inflammatory response in lung tissue triggered by radical radiotherapy. In a randomized, open-label Phase II study presented at ESMO 2024, prophylactic inhalation of beclomethasone propionate during radiotherapy was shown to significantly reduce the incidence of grade 3 or higher radiation pneumonitis compared to radiotherapy alone, with a lower overall incidence of all-grade radiation pneumonitis (35.3% vs. 52.8%).
