This combination therapy pairs becotatug vedotin (MK-2870/SKB264), a TROP2-directed antibody-drug conjugate (ADC), with ivonescimab (AK112), a bispecific antibody targeting PD-1 and VEGF. Becotatug vedotin utilizes a humanized monoclonal antibody to deliver a potent topoisomerase I inhibitor payload directly to TROP2-expressing cancer cells. Ivonescimab concurrently inhibits the PD-1/PD-L1 immune checkpoint and the VEGF-mediated angiogenic pathway, potentially creating a synergistic effect by enhancing T-cell infiltration and normalizing tumor vasculature. This specific combination is being evaluated by West China Hospital in a Phase II clinical trial as a neoadjuvant treatment for patients with resectable locally advanced head and neck squamous cell carcinoma (LAHNSCC) to improve pathological complete response rates and surgical outcomes.
