Overview

Bedaquiline is a first-in-class diarylquinoline antimycobacterial drug used in combination with other agents to treat pulmonary multidrug-resistant tuberculosis (MDR-TB) in adults and children aged 5 years and older who weigh at least 15 kg. It acts by inhibiting the mycobacterial ATP synthase enzyme, specifically binding to the c subunit of F-ATP synthase, which is essential for energy generation in Mycobacterium tuberculosis. This unique mechanism disrupts ATP production, leading to bacterial cell death. Bedaquiline is active against both drug-sensitive and drug-resistant strains of M. tuberculosis, including extensively drug-resistant TB (XDR-TB). The drug was developed by Janssen (a Johnson & Johnson company) and received accelerated approval from the US FDA in 2012 for MDR-TB when an effective regimen cannot otherwise be constructed[1][3][4][6][7].
