Overview

The combination of bedaquiline, delamanid, and linezolid is an investigational oral regimen for the treatment of multidrug-resistant (MDR) and extensively drug-resistant (XDR) tuberculosis. Each component has a distinct mechanism of action targeting Mycobacterium tuberculosis: - **Bedaquiline** inhibits mycobacterial ATP synthase, disrupting energy production. - **Delamanid** inhibits mycolic acid synthesis, impairing cell wall formation. - **Linezolid** inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. This combination is being studied as a potential backbone for shorter, more effective TB regimens. Preclinical studies in mouse models have shown that this regimen can significantly reduce treatment duration compared to standard therapies[1][3]. Clinical trials are ongoing or planned to evaluate its efficacy and safety in humans with MDR-TB or rifampicin-resistant TB[8]. The regimen is entirely oral and aims to improve outcomes while minimizing toxicity associated with older injectable drugs.
