This is a combination of two drugs: - **Bedaquiline** (formerly known as TMC207) is a diarylquinoline antimycobacterial agent that inhibits mycobacterial ATP synthase, an essential enzyme for energy production in *Mycobacterium tuberculosis*. It is primarily used for the treatment of multidrug-resistant tuberculosis (MDR-TB)[2][6][8]. Bedaquiline has a novel mechanism of action and was developed to address drug-resistant TB. - **Nevirapine** is a non-nucleoside reverse transcriptase inhibitor (NNRTI) used as part of antiretroviral therapy for HIV-1 infection. It acts by binding directly to reverse transcriptase and blocking RNA-dependent and DNA-dependent DNA polymerase activities[7]. The combination has been studied in the context of potential drug-drug interactions in patients co-infected with HIV-1 and tuberculosis, since both diseases often require concurrent therapy. Pharmacokinetic studies indicate that there are no clinically significant interactions between bedaquiline and nevirapine; they can be co-administered without dose adjustment[1][3][9]. The main clinical rationale for this combination is to enable safe concurrent treatment of MDR-TB with antiretroviral therapy containing nevirapine.
