Overview

**BE-PEP** is an investigational oral post-exposure prophylaxis regimen comprising **bedaquiline plus rifampicin** for prevention of leprosy among contacts of newly diagnosed leprosy cases. The regimen was developed by the Institute of Tropical Medicine as an enhanced alternative to WHO-recommended single-dose rifampicin prophylaxis. Bedaquiline inhibits mycobacterial ATP synthase, depleting cellular energy production, while rifampicin inhibits DNA-dependent RNA polymerase and thereby suppresses bacterial RNA synthesis. BE-PEP is being evaluated in the BE-PEOPLE program in the Comoros, including a Phase 3 cluster-randomized study comparing the regimen with standard single-dose rifampicin prophylaxis.
