Bekanamycin is an aminoglycoside antibiotic. It exerts its antibacterial effects by binding to ribosomal RNA (rRNA) in bacteria, specifically the A-site subdomain, thereby interfering with protein synthesis. It has also been computationally identified as a potential inhibitor of resuscitation-promoting factors B (RpfB) in *Mycobacterium tuberculosis*, suggesting a possible role in treating tuberculosis by preventing the reactivation of dormant bacteria. Beyond its antimicrobial actions, bekanamycin is known to have effects on mammalian systems, including depressing presynaptic calcium currents at motor nerve terminals, leading to neuromuscular blockade, and inhibiting slow calcium currents in cardiac pacemaker cells, which can decrease heart rate. It can also inhibit lysozyme activity and is associated with ototoxicity and nephrotoxicity, common side effects of aminoglycosides.
