Belantamab mafodotin + dexamethasone is an investigational combination therapy being studied for the treatment of primary immune thrombocytopenia (ITP). Belantamab mafodotin (Blenrep) is a first-in-class antibody-drug conjugate (ADC) targeting B-cell maturation antigen (BCMA), a protein highly expressed on plasma cells. It consists of a humanized IgG1 monoclonal antibody conjugated to the microtubule inhibitor monomethyl auristatin F (MMAF) via a non-cleavable maleimidocaproyl linker. By depleting BCMA-positive plasma cells, it aims to reduce the production of pathogenic autoantibodies in ITP. Dexamethasone is a potent glucocorticoid used to further suppress the immune response. This combination is currently being evaluated in a Phase II trial sponsored by the Hellenic Society of Hematology for adult patients with ITP who have failed prior treatments like TPO receptor agonists or rituximab.
