belantamab mafodotin + nirogacestat + pomalidomide + dexamethasone is an investigational quadruple drug regimen studied in patients with relapsed/refractory multiple myeloma. - **Belantamab mafodotin** is a first-in-class antibody-drug conjugate targeting B-cell maturation antigen (BCMA), delivering a cytotoxic agent directly into myeloma cells to induce cell death[3]. - **Nirogacestat** is a small molecule gamma-secretase inhibitor that increases cell-surface BCMA and reduces soluble BCMA, potentiating the anti-myeloma activity of BCMA-targeting agents[1][5]. - **Pomalidomide** is a third-generation immunomodulatory imide drug (IMiD) that enhances anti-tumor immunity in part by stimulating T cells and affecting the microenvironment[2][4]. - **Dexamethasone** is a synthetic glucocorticoid that inhibits inflammation and lymphocyte proliferation. The regimen is being tested in the DREAMM-5 platform trial to evaluate synergistic activity, dose, safety, and efficacy for patients with multiple myeloma who have failed prior lines of therapy[1][3][5].
