Belvarafenib + cobimetinib is an investigational combination therapy comprising **belvarafenib**, a potent and selective RAF dimer (type II) inhibitor, and **cobimetinib**, a selective MEK inhibitor. Both are orally administered small molecules. The combination was developed to more robustly suppress the MAPK pathway compared to single-agent therapy or BRAF/MEK inhibitor pairs, especially in tumors harboring RAS or RAF mutations, including *NRAS*-mutant melanoma. Clinical studies show the combination has acceptable tolerability and encouraging anti-tumor activity in NRAS-mutant melanoma, including in patients previously treated with immune checkpoint inhibitors. The combination aims to overcome resistance mechanisms observed in RAS/RAF-mutant cancers by dual inhibition at different nodes of the MAPK pathway. The combination is under clinical investigation, primarily for metastatic or locally advanced solid tumors with RAS or RAF mutations, with the principal focus to date on NRAS-mutant melanoma[1][3].
