Belvarafenib + cobimetinib + nivolumab is a combination investigational regimen for advanced or metastatic melanoma, specifically for patients harboring NRAS mutations and who have progressed on prior anti-PD-1/PD-L1 therapy. It consists of three agents: belvarafenib (a pan-RAF dimer inhibitor), cobimetinib (a MEK inhibitor), and nivolumab (a PD-1 immune checkpoint inhibitor). The combination is designed to block MAPK pathway signaling at multiple points (RAF and MEK) in tumor cells, while releasing immune inhibition via PD-1 blockade, with the aim of improving antitumor response and durability beyond single-agent regimens. The approach seeks to overcome resistance mechanisms in melanoma and provide benefit to patients with limited options. This combination is currently in clinical development for advanced melanoma and is not yet approved for other indications[3][4][5].
