Overview

A multi-agent chemoimmunotherapy regimen combining bendamustine, mitoxantrone, and rituximab used as salvage therapy for relapsed or refractory indolent non-Hodgkin lymphoma, including follicular lymphoma and mantle cell lymphoma. Bendamustine is an alkylating agent that cross-links DNA causing impaired DNA replication and repair; mitoxantrone is an anthracenedione that intercalates DNA and inhibits topoisomerase II, leading to DNA breaks; rituximab is an anti-CD20 monoclonal antibody that induces B-cell depletion via complement-dependent cytotoxicity, antibody-dependent cellular cytotoxicity, and direct apoptosis. Phase II studies reported high overall response rates and manageable toxicity for the bendamustine–mitoxantrone–rituximab regimen in relapsed/refractory indolent lymphoma, and variants including dexamethasone (RBMD) have also shown efficacy in follicular lymphoma.[4][5][6]
