This combination therapy consists of the alkylating agent bendamustine, the immunomodulatory drug pomalidomide, and the PD-1 checkpoint inhibitor sintilimab. It is primarily investigated for the treatment of relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL). Bendamustine acts as a cytotoxic agent by inducing DNA cross-linking and subsequent apoptosis. Pomalidomide, a thalidomide analogue, binds to the cereblon E3 ubiquitin ligase complex, leading to the degradation of transcription factors Ikaros and Aiolos, which enhances T-cell and NK-cell mediated immunity while directly inhibiting tumor cell growth. Sintilimab is a monoclonal antibody that blocks the interaction between the PD-1 receptor on T-cells and its ligands, thereby restoring the anti-tumor immune response. The triplet regimen aims to leverage the synergistic effects of direct cytotoxicity, immunomodulation, and checkpoint blockade to overcome resistance in aggressive lymphomas.
