Benzathine penicillin G is a long-acting, injectable antibiotic in the natural penicillins class. It is used to treat and prevent a variety of bacterial infections caused by susceptible organisms, including mild-to-moderate upper respiratory tract infections, syphilis, yaws, bejel, pinta, and to prevent rheumatic fever and related conditions. The drug consists of the combination of benzathine (which slows absorption) with penicillin G (benzylpenicillin), resulting in prolonged low serum levels after intramuscular injection. Its mechanism of action involves inhibiting bacterial cell wall synthesis by binding to and inactivating transpeptidase enzymes involved in peptidoglycan cross-linking during cell wall biosynthesis. This leads to osmotic instability and lysis of susceptible bacteria[1][2][7][8]. It is not effective against viral infections or for use in patients with known hypersensitivity to penicillins.
