Overview

Benzocaine + lidocaine + epinephrine is a combination anesthetic regimen evaluated by the David Grant U.S. Air Force Medical Center for the management of procedural pain during cosmetic facial filler injections (e.g., Juvederm XC). The protocol involves the topical application of 20% benzocaine gel as a pre-treatment, followed by a nerve block injection of 2% lidocaine with 1:100,000 epinephrine. Benzocaine and lidocaine are local anesthetics that inhibit nerve conduction by blocking voltage-gated sodium channels. Epinephrine is included as a vasoconstrictor to prolong the local anesthetic effect and minimize systemic absorption. This specific combination was studied in a Phase 4 clinical trial (NCT02154399) to compare its efficacy against topical 4% Topicaine gel in patients undergoing lip or labiomental fold augmentation.
