Overview

Benzoyloxymethylthiamine is a lipophilic derivative and prodrug of thiamine (Vitamin B1). It was developed to enhance the bioavailability and tissue penetration of thiamine compared to standard water-soluble forms like thiamine hydrochloride. Upon administration, the compound is metabolized to release free thiamine, which is then phosphorylated to thiamine pyrophosphate (TPP), a vital cofactor for enzymes such as pyruvate dehydrogenase and transketolase. These enzymes are crucial for aerobic metabolism and energy production, particularly within the nervous system. Clinical research, primarily conducted in the late 1980s, demonstrated its efficacy in reducing the intensity and duration of postoperative pain following dental surgeries, such as third molar extraction, suggesting a role for high-dose thiamine derivatives in managing acute inflammatory pain and supporting nerve recovery.
