Benztropine is a synthetic anticholinergic and antihistaminic agent primarily used as an adjunct in the management of all forms of parkinsonism, including idiopathic Parkinson's disease and parkinsonism resulting from encephalitis or drug-induced extrapyramidal symptoms. Its main mechanism involves blocking muscarinic acetylcholine receptors (especially M1 subtype) in the central nervous system, particularly within the striatum, thereby restoring the balance between dopamine and acetylcholine. Benztropine also inhibits presynaptic dopamine reuptake, increasing synaptic dopamine levels, and exhibits antihistaminic activity. It is structurally related to both atropine and diphenhydramine. The drug is available as oral tablets and injectable solutions.
