Besilesomab is a murine (mouse-derived) monoclonal antibody of the IgG1 isotype that is radiolabelled with technetium-99m for use as a diagnostic radiopharmaceutical. It specifically binds to nonspecific cross-reacting antigen 95 (NCA-95), also known as CD66 or CEACAM8, which is expressed on the surface of granulocytes and their precursors. By targeting these antigens, besilesomab accumulates at sites of infection or inflammation where granulocytes are present. Its primary clinical use is in scintigraphic imaging to localize inflammation or infection in peripheral bone in adults with suspected osteomyelitis. Besilesomab does not activate complement or affect granulocyte function and has no clinically relevant pharmacodynamic effects at recommended doses. It has been approved for diagnostic use in several European countries under the brand name Scintimun[1][2][4][5][6].
