beta-Hydroxythiofentanyl is a synthetic opioid analgesic and a structural analog of fentanyl and thiofentanyl. It acts as an **agonist** at the µ-opioid receptor, producing effects similar to fentanyl, including potent analgesia, euphoria, and significant risk of respiratory depression. The compound has no accepted medical use in the United States or most other countries and is classified as a Schedule I controlled substance due to its high potential for abuse and lack of safety under medical supervision. It has been encountered on the illicit market since the mid 1980s, often sold as a powder or tablet—sometimes mixed with or misrepresented as heroin—and has been associated with fatal overdoses[1][3][5]. Like other fentanyl analogs, its side effects include itching, nausea, and life-threatening respiratory depression[3][5]. There are no approved therapeutic indications for this drug.
