Overview

This is a combination therapy consisting of a beta-blocker and ivabradine, used primarily for the management of chronic stable angina pectoris and heart failure with reduced ejection fraction (HFrEF) in patients whose symptoms are not adequately controlled by beta-blockers alone. Beta-blockers act as antagonists at beta-adrenergic receptors, reducing heart rate, myocardial contractility, and blood pressure. Ivabradine selectively inhibits the hyperpolarization-activated cyclic nucleotide-gated (HCN) channel responsible for the cardiac pacemaker "funny" current (If), leading to further reduction in heart rate without affecting myocardial contractility or blood pressure. The combination is effective in reducing anginal symptoms, lowering heart rate, decreasing nitrate consumption, improving quality of life in angina patients[1][2], and reducing hospitalizations due to worsening heart failure[3][4][5]. It is generally well tolerated; bradycardia risk increases but remains low when appropriately monitored.
