**Betaine + polyhexanide** is a topical wound-cleansing and antimicrobial combination marketed by B. Braun as **Prontosan**. It contains the surfactant undecylenamidopropyl betaine and the polymeric biguanide antiseptic polyhexanide, also called polyhexamethylene biguanide or PHMB. Betaine lowers surface tension and helps loosen wound debris, slough, and biofilm, while polyhexanide exerts broad antimicrobial activity through disruption of microbial cell membranes. Prontosan is supplied as wound irrigation solution and gel for cleansing and moistening acute and chronic wounds, including ulcers, abrasions, postsurgical wounds, and burns. In the United States, Prontosan wound products are FDA-cleared wound-management devices rather than FDA-approved prescription drugs.
