betamethasone + bupivacaine is a combination therapy utilized for the management of Subacromial Impingement Syndrome (SIS). It consists of betamethasone, a potent synthetic glucocorticoid with anti-inflammatory and immunosuppressive properties, and bupivacaine, a long-acting local anesthetic of the amide type. Betamethasone acts by binding to intracellular glucocorticoid receptors, leading to the suppression of pro-inflammatory cytokines and reduction of mechanical repetitive inflammation in the supraspinatus tendon and subacromial bursa. Bupivacaine provides regional analgesia by blocking voltage-gated sodium channels, thereby preventing the generation and conduction of nerve impulses. This specific combination was evaluated in a Phase 4 clinical trial (NCT05114356) sponsored by Marmara University to facilitate patient participation in supervised exercise programs by providing immediate and prolonged pain relief.
