Overview

Betulin is a naturally occurring pentacyclic triterpene of the lupane type, most abundantly found in the bark of birch trees (genus Betula), where it can constitute up to 30% of the dry weight. It is also present in birch sap and some fungi such as Inonotus obliquus. Chemically, it features a pentacyclic ring structure with hydroxyl groups at C3 and C28[7]. Betulin exhibits a broad spectrum of biological activities including anti-inflammatory, anticancer (antineoplastic), antiviral, antibacterial, antifungal, analgesic, immune regulatory and wound-healing effects[1][3][6]. Its mechanism of action involves modulation of multiple molecular targets and pathways: - Inhibition of proinflammatory cytokines (IL-6, IL-1β, TNFα) via downregulation of NFκB signaling - Modulation/inhibition of MAPK pathway - Induction of apoptosis through mitochondrial damage and activation of caspase 9/3 - Enhancement of antioxidant defense by increasing HO-1/Nrf2 expression and glutathione S-transferase activity Betulin also inhibits enzymes such as α-amylase and α-glucosidase (relevant for diabetes) and has been shown to reduce COX-2 activity in some derivatives[1][6][8]. Most research on betulin focuses on its potential as an investigational agent for cancer therapy or anti-inflammatory applications; however its poor solubility limits clinical development so far.
