Overview

This is a four-drug combination regimen consisting of bevacizumab, carboplatin, everolimus, and paclitaxel. Each component has a distinct mechanism of action: - **Bevacizumab** is a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis (the formation of new blood vessels) essential for tumor growth. - **Carboplatin** is a platinum-based chemotherapeutic agent that induces DNA crosslinking and apoptosis in rapidly dividing cells. - **Everolimus** is an mTOR inhibitor that blocks the mammalian target of rapamycin pathway, inhibiting cell proliferation and angiogenesis. - **Paclitaxel** is a microtubule-stabilizing agent that disrupts mitosis by preventing microtubule depolymerization. This combination has been studied primarily in advanced or metastatic melanoma. In randomized phase II trials, the addition of everolimus to the established regimen of carboplatin, paclitaxel, and bevacizumab did not improve progression-free survival or overall survival but increased toxicity. The regimen demonstrates moderate antitumor activity with notable adverse effects including neutropenia, leukopenia, fatigue, hypertension, thrombocytopenia, and gastrointestinal events[1][2][5].
