This is a combination regimen consisting of five agents used in the treatment of metastatic colorectal cancer (mCRC). The regimen includes two monoclonal antibodies—bevacizumab, which targets vascular endothelial growth factor (VEGF), and cetuximab, which targets epidermal growth factor receptor (EGFR)—along with three chemotherapeutic agents commonly used in colorectal cancer therapy: fluorouracil (a pyrimidine analog antimetabolite that inhibits thymidylate synthase), irinotecan (a topoisomerase I inhibitor), and leucovorin (folinic acid, which enhances the efficacy of fluorouracil). This combination aims to inhibit tumor angiogenesis and cell proliferation through multiple mechanisms. It has been studied primarily in heavily pretreated patients with mCRC, particularly those who have progressed on standard regimens. While preclinical data suggested potential synergy between anti-VEGF and anti-EGFR therapies, clinical trials have shown mixed results regarding efficacy and toxicity when combining all these agents[2][3][6][7].
