This is a combination regimen consisting of three agents: - **Bevacizumab** is a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis and reducing tumor blood supply. - **Irinotecan** is a small molecule topoisomerase I inhibitor, which interferes with DNA replication in cancer cells, leading to cell death. It is commonly used in metastatic colorectal cancer (mCRC) as part of the FOLFIRI regimen. - **Silymarin** is a flavonolignan complex extracted from milk thistle (Silybum marianum), often used as a dietary supplement for its hepatoprotective and antioxidant properties. In this context, silymarin has been studied as an adjunct to reduce chemotherapy-induced toxicities—particularly diarrhea and nausea—without impairing antitumor efficacy. The combination has been investigated primarily in mCRC patients receiving first-line FOLFIRI plus bevacizumab regimens, where silymarin supplementation was associated with reduced rates of diarrhea and nausea compared to standard therapy alone[1][2][4]. Silymarin may exert additional effects by inhibiting bacterial β-glucuronidase activity (reducing SN-38 reactivation in the gut), modulating VEGF expression, STAT3 signaling, c-Myc pathways, and providing antioxidant support[6].
