Overview

This is a combination regimen consisting of the monoclonal antibody bevacizumab, the modified FOLFOX6 chemotherapy protocol (which includes folinic acid [leucovorin], fluorouracil [5-FU], and oxaliplatin), and the small molecule multikinase inhibitor sorafenib. - **Bevacizumab** is a recombinant humanized monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis in tumors. - **mFOLFOX6** is a standard chemotherapy regimen for colorectal and other gastrointestinal cancers; it combines: - Folinic acid (leucovorin): enhances binding of 5-FU to thymidylate synthase. - Fluorouracil (5-FU): an antimetabolite that inhibits DNA synthesis by targeting thymidylate synthase. - Oxaliplatin: a platinum-based agent causing DNA crosslinking and apoptosis. - **Sorafenib** is an oral small molecule inhibitor targeting multiple kinases including RAF kinases, VEGFR, PDGFR, FLT3, c-KIT, and RET; it blocks tumor cell proliferation and angiogenesis. This combination targets both tumor cell division directly via cytotoxic agents as well as tumor vasculature through antiangiogenic mechanisms. It has been investigated primarily in advanced or metastatic colorectal cancer settings.
