This is a three-drug combination regimen consisting of bevacizumab, octreotide long-acting release (LAR), and capecitabine. Bevacizumab is a humanized monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis. Octreotide LAR is a somatostatin analog that binds to somatostatin receptors, inhibiting the secretion of various hormones and peptides involved in tumor growth. Capecitabine is an oral prodrug of 5-fluorouracil (5-FU) that interferes with DNA synthesis by inhibiting thymidylate synthase, leading to cytotoxic effects on rapidly dividing cells. The combination has been studied primarily for the treatment of metastatic well-to-moderately differentiated neuroendocrine neoplasms (WMD-NEN). The XELBEVOCT regimen was evaluated in phase 2 clinical trials and demonstrated activity with manageable toxicity profiles. The rationale for this combination includes antiangiogenic effects from both bevacizumab and metronomic capecitabine, as well as antiproliferative and hormone-inhibitory actions from octreotide LAR[1][2][4].
