Overview

The combination of intravitreal bevacizumab (IVB) and topical dorzolamide eye drops has been investigated as a treatment approach for diabetic macular edema (DME). Bevacizumab is an anti-VEGF (vascular endothelial growth factor) monoclonal antibody administered via intravitreal injection, while dorzolamide is a carbonic anhydrase inhibitor typically used topically for glaucoma. Clinical trials have evaluated whether this combination provides additional benefits over bevacizumab alone for DME. Results from a randomized double-masked clinical trial indicated that adjuvant topical dorzolamide with IVB injection had no additional significant effects on best-corrected visual acuity (BCVA), central macular thickness (CMT), or central macular volume (CMV) compared to IVB alone over a three-month course. The theoretical rationale includes dorzolamide's potential to decrease aqueous production and outflow, which might slow the clearance of intravitreal drugs like bevacizumab, and its general utility in treating macular edema of various etiologies.
