Bevonescein (ALM-488) is a fluorescent peptide-dye conjugate developed by Alume Biosciences for real-time intraoperative nerve visualization. It is designed to highlight nerves during surgical procedures, such as head and neck surgeries (parotidectomy, thyroidectomy), to prevent accidental nerve injury. Unlike traditional methods, bevonescein binds to the extracellular matrix of nerves rather than myelin, allowing it to illuminate motor, sensory, autonomic, and even degenerated nerves. Administered intravenously approximately 60 minutes before surgery, it provides a fluorescence overlay on white light reflectance, enabling surgeons to identify and preserve critical nerve structures. It has received FDA Fast Track designation and is currently in Phase 3 clinical trials, with a New Drug Application (NDA) submission planned for 2026.
