Bexarotene + methoxsalen is a combination therapy primarily utilized in the management of advanced or refractory cutaneous T-cell lymphoma (CTCL), including mycosis fungoides and Sézary syndrome. Bexarotene is a synthetic retinoid that selectively activates retinoid X receptors (RXRs), which function as transcription factors regulating cell differentiation and apoptosis. Methoxsalen is a photoactive compound (psoralen) that, when activated by ultraviolet A (UVA) light (PUVA therapy) or used in extracorporeal photopheresis (ECP), forms DNA cross-links to inhibit cell proliferation and induce apoptosis in malignant T-cells. The combination is often employed to enhance clinical response in patients who have failed monotherapy, leveraging the synergistic effects of RXR activation and DNA-targeted photochemotherapy.
