Bexmarilimab + anti-PD-1 is a combination immunotherapy being developed by Faron Pharmaceuticals to treat advanced solid tumors, specifically targeting patients who have developed resistance to prior PD-1/PD-L1 inhibitors. Bexmarilimab is a humanized monoclonal antibody that targets **Clever-1** (Common lymphatic endothelial and vascular endothelial receptor 1, also known as Stabilin-1), a scavenger receptor expressed on tumor-associated macrophages (TAMs). By inhibiting Clever-1, bexmarilimab aims to reprogram immunosuppressive M2 macrophages into immune-stimulating M1 macrophages, thereby removing the 'brakes' on the immune system within the tumor microenvironment. In the Phase 1/2 **BLAZE** trial, bexmarilimab is combined with **zimberelimab**, a PD-1 inhibitor provided by Gilead Sciences, to reinvigorate T-cell activity in patients with PD-1 resistant non-small cell lung cancer (NSCLC) and melanoma. The treatment strategy often involves a 7-day priming period with bexmarilimab monotherapy before starting the combination regimen.
