Overview

Bezafibrate is a lipid-lowering agent belonging to the fibrate class of drugs. It is primarily used to treat hyperlipidaemia by lowering elevated levels of cholesterol and triglycerides in the blood. The drug works by decreasing low-density lipoproteins (LDL) and very-low-density lipoproteins (VLDL), while increasing high-density lipoprotein (HDL) levels. Its mechanism involves increasing the activity of triglyceride lipases such as lipoprotein lipase and hepatic lipoprotein lipase—enzymes that promote the catabolism of triglyceride-rich lipoproteins—and reducing cholesterol biosynthesis with stimulation of LDL-receptor-mediated catabolism. Clinically approved since 1978 and patented in 1971[5], bezafibrate is indicated for primary and secondary hyperlipidaemia when lifestyle modifications are insufficient or statins cannot be used[2][5]. It may also be combined with statins if monotherapy fails to achieve lipid targets[1]. Additional research suggests potential benefits in metabolic syndrome and diabetes prevention due to its effects on glucose metabolism markers[5].
