Bezafibrate + ursodeoxycholic acid is a combination therapy used primarily for the treatment of primary biliary cholangitis (PBC), especially in patients who have an inadequate response to ursodeoxycholic acid (UDCA) monotherapy. Bezafibrate is a fibrate class lipid-lowering agent that acts as an agonist of peroxisome proliferator-activated receptor-alpha (PPAR-α), leading to improved lipid metabolism and modulation of bile acid synthesis. Ursodeoxycholic acid is a hydrophilic bile acid that improves bile flow, reduces toxic bile acids, and has cytoprotective effects on hepatocytes. The combination has been shown in multiple randomized controlled trials and meta-analyses to significantly improve liver biochemistries—particularly alkaline phosphatase (ALP) and gamma-glutamyltransferase (GGT)—in PBC patients with incomplete response to UDCA alone[1][2][4][5][6]. However, its effect on mortality, liver-related complications, or quality of life remains unclear[1][6]. Bezafibrate may also reduce pruritus associated with PBC[8].
