Bezisterim (NE3107) is an investigational, orally bioavailable small molecule being developed by BioVie for the treatment of Long COVID, Alzheimer's disease, and Parkinson's disease. It acts as an anti-inflammatory and insulin-sensitizing agent that effectively crosses the blood-brain barrier. Its primary mechanism involves inhibiting Toll-like receptor 4 (TLR-4) driven activation of nuclear factor kappa B (NFκB) and modulating extracellular signal-regulated kinase (ERK) signaling. By targeting these pathways, bezisterim reduces the expression of pro-inflammatory cytokines such as IL-6, TNF, and IFNγ without being immunosuppressive. In Long COVID, it is specifically being evaluated for its potential to alleviate neurocognitive symptoms ("brain fog") and fatigue.
