Overview

Bezlotoxumab + fidaxomicin is a combination therapy used for the management of Clostridioides difficile infection (CDI), particularly in patients at high risk for recurrence. Bezlotoxumab is a humanized immunoglobulin G1 monoclonal antibody that targets and neutralizes C. difficile toxin B, thereby reducing damage to colon cells and lowering recurrence rates when administered alongside standard antibiotics. Fidaxomicin is a narrow-spectrum macrolide antibiotic that inhibits bacterial RNA polymerase, leading to the death of C. difficile with minimal disruption to normal gut flora. The combination aims to both treat active infection (fidaxomicin) and prevent recurrence (bezlotoxumab). Clinical studies suggest this combination may be especially beneficial in patients with multiple risk factors for CDI relapse, though real-world data are limited and cost considerations are significant[5][7][8].
