BFM-90 is not a single drug but a multi-agent chemotherapy protocol developed by the Berlin-Frankfurt-Münster (BFM) group for the treatment of acute lymphoblastic leukemia (ALL) and mature B-cell neoplasms in children, adolescents, and young adults. The regimen consists of several chemotherapeutic agents administered in specific sequences and doses over multiple phases (induction, consolidation/intensification, maintenance). Key drugs commonly included are prednisone or dexamethasone (glucocorticoid), vincristine (vinca alkaloid), daunorubicin or doxorubicin (anthracycline), L-asparaginase or PEG-asparaginase (enzyme), cyclophosphamide (alkylating agent), cytarabine/ara-C and methotrexate (antimetabolites), etoposide (topoisomerase inhibitor), mercaptopurine/6-MP and thioguanine/6-TG. The protocol may also include intrathecal chemotherapy for CNS prophylaxis. The regimen is risk-adapted based on patient characteristics such as age, white blood cell count at diagnosis, response to initial therapy, cytogenetics/molecular markers, and disease stage[2][3][5][6]. It has been widely used internationally as a standard-of-care pediatric-inspired protocol for ALL in both pediatric and adolescent/young adult populations.
