BG9588 + IDEC 131 is a combination of two distinct **humanized monoclonal antibodies** targeting **CD154 (CD40 ligand, CD40L)**, designed as immunosuppressive agents for autoimmune diseases and transplantation. - **BG9588** (also known as **ruplizumab**) and **IDEC 131** (also known as **toralizumab**) both bind to CD154, blocking the interaction with CD40, a critical co-stimulatory pathway in T cell activation and B cell function. - The principal indications explored for both antibodies included systemic lupus erythematosus (SLE), particularly lupus nephritis, and other autoimmune diseases. - Clinical trials demonstrated serological and renal activity improvements for BG9588, but both drugs were discontinued due to increased incidence of **thromboembolic events** (e.g., myocardial infarction). - Developed primarily in the late 1990s and early 2000s, both products were discontinued after Biogen and Idec merged due to safety concerns specific to the anti-CD40L mechanism[1][3][4][5][7].
