BGB-B167 is a first-in-class, IgG-based bispecific antibody developed by BeiGene for the treatment of advanced or metastatic solid tumors. It simultaneously targets carcinoembryonic antigen-related cell adhesion molecule 5 (CEACAM5, also known as CEA), which is overexpressed in many cancers, and the T-cell co-stimulatory receptor 4-1BB (CD137), found on activated CD4+ and CD8+ T lymphocytes. By crosslinking CEA-expressing tumor cells with 4-1BB-expressing immune cells, BGB-B167 locally activates T-cell responses within the tumor microenvironment while minimizing systemic toxicity. Preclinical studies have shown potent antitumor activity and immune cell activation dependent on CEA presence, with a favorable safety profile compared to other agents targeting similar pathways. Clinical development has focused on patients with unresectable or metastatic colorectal cancer (CRC), gastric cancer (GC), and non-small cell lung cancer (NSCLC) expressing CEA who have progressed after standard therapies[1][3][6].
