Overview

BGC20-1259 is a small molecule inhibitor of 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1) originally developed by BTG (British Technology Group). The drug was primarily investigated for the treatment of Alzheimer's disease and metabolic disorders such as type 2 diabetes. By selectively inhibiting 11β-HSD1, BGC20-1259 prevents the conversion of inactive cortisone into active cortisol within specific tissues, including the brain and liver. In the context of Alzheimer's, reducing local cortisol levels is hypothesized to mitigate glucocorticoid-induced neurotoxicity and improve cognitive function. Development reached Phase 1 clinical trials, but the program appears to have been discontinued as BTG shifted its strategic focus toward interventional medicine.
