Overview

BHQ880 is a fully human monoclonal antibody that targets and neutralizes Dickkopf-related protein 1 (DKK1), a potent antagonist of the Wnt signaling pathway. By binding to and inhibiting DKK1, BHQ880 enhances Wnt pathway signaling, which promotes osteoblast differentiation and activation within the bone matrix. This mechanism may help reverse tumor-induced osteolytic disease commonly seen in multiple myeloma. Preclinical studies have shown that BHQ880 upregulates β-catenin levels while downregulating NF-κB activity in bone marrow stromal cells. Clinically, it has demonstrated antitumor activity and inhibition of myeloma cell growth in both minimal disease and high-tumor-burden models. Developed by Novartis and MorphoSys (now a subsidiary of Novartis), it has reached phase II clinical trials for multiple myeloma—including patients with renal insufficiency—and smoldering multiple myeloma[1][2][3][6].
