Overview

BHV-1400 is a first-in-class, novel antibody-based protein degrader developed by Biohaven for the treatment of IgA nephropathy (IgAN). It is designed as a bifunctional conjugate that selectively binds and depletes galactose-deficient IgA1 (Gd-IgA1), the pathogenic antibody driving IgAN, via ASGPR-mediated hepatocyte internalization. This targeted removal of aberrant Gd-IgA1 occurs rapidly and deeply—Phase 1 studies have shown up to 81% reduction in Gd-IgA1 within hours after subcutaneous administration, with effects sustained for weeks. Importantly, BHV-1400 spares healthy immunoglobulins (IgA, IgG, IgE, and IgM) and does not cause clinically significant changes in innate or adaptive immunity. The drug represents a non-immunosuppressive approach to disease modification in patients with IgAN[3][4][5][6][9].
