BI 1034020 is a bispecific Nanobody® developed for the treatment of Alzheimer's disease. It was designed to target two different epitopes of amyloid beta (Aβ), specifically Aβ40 and Aβ42, with high affinity. The drug's mechanism involves inhibiting the beta amyloid A4 protein (APP), aiming to reduce free Aβ peptide levels in plasma, prevent new plaque formation, and clear existing plaques in the brain. The originator of this molecule is Ablynx, with Boehringer Ingelheim Pharmaceuticals as a key developer. Clinical development reached phase I but was terminated prematurely due to a serious adverse event during early testing in healthy volunteers[1][2][3][5][8].
