BI 1821736 is a genetically engineered oncolytic virus derived from vesicular stomatitis virus (VSV) with a modified glycoprotein (GP). It is designed as an immunotherapy for cancer and functions by preferentially replicating in and destroying tumor cells that have defective or inhibited type I interferon signaling—a common feature of many cancers. The immune-stimulatory cargo of the virus leads to T-cell priming and reactivation within the immunosuppressed tumor microenvironment, thereby enhancing anti-tumor immune responses. Preclinical studies have shown dose-dependent inhibition of tumor growth. The drug is being developed primarily for advanced solid tumors and has been evaluated in first-in-human Phase I clinical trials as an intravenous infusion every three weeks.
