BI 187004 + metformin is a combination therapy consisting of BI 187004 (an oral 11β-hydroxysteroid dehydrogenase-1 (11β-HSD1) inhibitor) and metformin (a standard antidiabetic medication). This combination was investigated for the treatment of type 2 diabetes mellitus (T2DM) in patients with overweight or obesity. BI 187004 works by inhibiting the 11β-HSD1 enzyme, which was hypothesized to have positive effects on multiple cardiovascular and metabolic risk factors associated with metabolic syndrome. However, clinical trials showed that despite complete 11β-HSD1 inhibition, the combination did not demonstrate clinically relevant effects on glucose metabolism or weight loss parameters.
