BI 3812465 is a small molecule inhibitor of the Transient Receptor Potential Canonical 6 (TRPC6) channel, developed by Boehringer Ingelheim for the treatment of center-involved diabetic macular edema (DME). TRPC6 is a non-selective cation channel that plays a critical role in regulating vascular permeability and calcium signaling within the retina. Overactivation of TRPC6 is linked to the breakdown of the blood-retinal barrier, leading to the fluid accumulation characteristic of DME. By selectively antagonizing TRPC6, BI 3812465 aims to stabilize retinal vascular integrity and reduce edema. The drug is administered via intravitreal injection and has been evaluated in Phase I/IIa clinical trials to assess its safety, tolerability, and efficacy in patients with DME.
