Overview

BI 409306 + donepezil is a combination therapy that has been investigated in clinical trials for Alzheimer's disease. This combination pairs BI 409306, a phosphodiesterase 9A (PDE9A) inhibitor developed by Boehringer Ingelheim, with donepezil, an established acetylcholinesterase inhibitor commonly used in Alzheimer's treatment[1][5][6]. ## Mechanism of Action BI 409306 works by inhibiting PDE9A, an enzyme that reduces brain levels of cyclic guanosine monophosphate (cGMP)[5]. By inhibiting this enzyme, BI 409306 aims to increase cGMP levels, which should theoretically improve synaptic transmission and plasticity in the hippocampus and cerebral cortex[5]. The compound has been shown to enhance long-term potentiation (LTP) and was hypothesized to improve cognitive function and memory by strengthening synaptic plasticity[4]. Donepezil, on the other hand, is an acetylcholinesterase inhibitor that works by preventing the breakdown of acetylcholine in the brain, thereby increasing its concentration and enhancing cholinergic neurotransmission[6]. This mechanism has been proven effective in improving cognitive function in patients with mild to moderate Alzheimer's disease. ## Clinical Development The combination of BI 409306 and donepezil has been studied in clinical trials to investigate potential drug-drug interactions and therapeutic efficacy[1][2][3]. According to the search results, a pharmacokinetic drug-drug interaction study was conducted to evaluate the effects of co-administering donepezil 10 mg with BI 409306 25 mg[1]. The results indicated that co-administration had no clinically relevant effect on the exposure of BI 409306[1]. In addition, BI 409306 was evaluated in two Phase 2 proof-of-concept clinical trials for cognitive impairment associated with prodromal Alzheimer's disease and mild Alzheimer's disease[4]. In one of these trials, some patients received BI 409306 as an add-on to donepezil[5]. However, the results from these trials were disappointing, with Boehringer Ingelheim reporting in February 2018 that there was no difference between drug and placebo groups on measures of efficacy[5][9]. Despite the negative results in Alzheimer's disease trials, Boehringer Ingelheim continued to investigate BI 409306 for schizophrenia, exploring whether it could prevent a first psychotic episode or prevent relapse[5]. The development status of this combination therapy for Alzheimer's disease appears to be discontinued based on the negative Phase 2 results reported in 2018[5].
