Overview

BI-443651 is a small molecule inhibitor of the epithelial sodium channel (ENaC), developed for the treatment of cystic fibrosis. By blocking ENaC, it aims to reduce sodium and fluid absorption in airway epithelia, thereby improving airway hydration and mucociliary clearance—key therapeutic goals in cystic fibrosis. The drug has demonstrated a favorable safety profile in phase 1 clinical trials, with studies showing good tolerability in both healthy volunteers and individuals with mild asthma or cystic fibrosis[2][5][8]. Boehringer Ingelheim is the developer of this compound[3][4]. As an ENaC antagonist, its mechanism is distinct from CFTR modulators and may offer benefits as monotherapy or adjunctive therapy for patients regardless of their CFTR mutation status.
