BI 765063 is a first-in-class, humanized IgG4 monoclonal antibody that acts as an antagonist of signal regulatory protein alpha (SIRPα), a key myeloid cell checkpoint inhibitor. By binding selectively to the V1 allele of SIRPα and blocking its interaction with CD47 (the "don't eat me" pathway), BI 765063 prevents inhibitory signaling in myeloid cells such as macrophages and dendritic cells. This blockade enhances anti-tumor immune responses by promoting phagocytosis and modifying the tumor microenvironment. The drug is being developed for use in advanced solid tumors—including colorectal cancer and hepatocellular carcinoma—primarily under collaboration between OSE Immunotherapeutics and Boehringer Ingelheim.
