BI 836826 + venetoclax is a combination investigational regimen consisting of BI 836826, a chimeric IgG1 monoclonal antibody targeting CD37 (a transmembrane protein highly expressed on normal and malignant B-cells), and venetoclax, a small molecule inhibitor of B-cell lymphoma-2 (BCL-2). BI 836826 activates both antibody-dependent cell-mediated cytotoxicity and has intrinsic proapoptotic activity against malignant B cells, while venetoclax selectively inhibits BCL-2, restoring apoptosis in chronic lymphocytic leukemia (CLL) and other B-cell malignancies. The combination is being studied primarily in patients with relapsed/refractory CLL, aiming to achieve synergistic clearance of malignant B cells and higher rates of complete remission and minimal residual disease negativity compared to single-agent or alternative regimens[1][3][4][9].
